Physician House Call Services - Medical Marijuana Evaluation and Certification

We offer home visit services to aid homebound patients at private residences, senior housing, assisted living, extended care, and hospice care facilities throughout the state of CT. $385 for new patients and $350 for renewal patients within 15 miles of our Milford office, add $5 for every extra mile each way from our office to your home address (physician house call total fee for the most distant locations in CT is not to exceed $585)
