our office policies:
Payment is due at the time of the appointment. We accept cash, all major debit and credit cards.
Connecticut law does not require health insurers to cover medical marijuana related costs. We, therefore, do not bill insurance companies.
Please arrive 15 minutes before your appointment if the pre-visit electronic forms are completed, or at least 45 minutes before if the forms are not completed. This provides adequate time for check-in and processing of submitted documents.
We understand that delays can happen, however, we must try to keep other patients and our staff on time. If you are more than 15 minutes late to your scheduled appointment, we may have to reschedule your visit for another time.
Missed appointments and late cancellations
We do not double or triple book our appointments. The time allocated for your appointment is the time reserved for you. In consideration to other patients and our staff, please keep your appointment as scheduled. If you cannot make your appointment, please notify us at least 48 hours before your appointment time. Missed/no-show appointments and any changes made less than 48 hours before the appointment will require a prepayment of your future appointment fee in order to reserve an alternative appointment.
One time rescheduling is allowed, if it is done at least 48 hours before your appointment time. Repeated rescheduling will require a prepayment of your future appointment fee in order to reserve an alternative appointment.
Scheduling back-to-back appointments requires a $50 non-refundable deposit that is applied toward the appointment fee.
NOTE: In the event a patient does not register with the state of Connecticut Department of Consumer Protection Medical Marijuana Program after the certification by the physician or does not provide any document required for the certification for any reason within four (4) months or longer from the date of the initial evaluation, a follow-up appointment will be required to reassess the patient’s current condition before certifying/re-certifying the patient to the state program.
No Outside Food or Beverage Please
In order to keep the office neat and clean for all of our patients, no outside food or beverage is allowed in the waiting or treatment areas. Thank you for understanding.
